Subject: ESAF for work in Polymer Lab 401/L2104 (spin coater) 
The TRR Polymer Lab L2104 in Building 401, has a spin coater in an exhaust hood under ambient conditions. Users planning to work in the polymer lab should submit an ESAF prior to doing the work, according to the following procedure.

Start from the ESAF home page:

log in and click the link at the top 'Create a New No X-Ray ESAF'

An ESAF form appears, but with only the General tab. Fill out the General tab as usual. Three items on the General tab have special instructions. (1) In the Sector field, select "81-XSD-TRR" from the drop-down menu, (this number "81" could change, it used to be 97, currently [Sept 2017] it is 81). (2) In the ESAF Type field, "No X-ray". (3) For Techniques, choose "No X-Ray".

When you click the Save button on the General tab, the other tabs will appear. Fill out all the details as ususal, including the details of the work procedures, required chemicals, and the wastes generated.
On the Description tab, for "location for non-Xray work", enter "401/L2104".

A Description might be along these lines:
We will use the commercial spin coater unit available in this lab to deposit solutions of [POLYMER] dissolved in [SOLVENT] onto silicon pieces approx. 20 mm x 20 mm x 0.5 mm. We will wear safety glasses and impervious gloves while handling the solution and thin film samples. Typically, each deposition requires [< 100 uL] of solution. The spin coater is located inside a ventilated hood. The solution [IS NOT/IS] stored in a vial that [IS NOT/IS] sealed with a septum so we [WILL NOT/WILL] use a syringe with a hypodermic needle for handling the solution. We [WILL NOT/WILL] be handling sharps and [WILL NOT/WILL] generate sharps waste. [DESCRIBE ANY SPECIAL HAZARDS ASSOCIATED WITH YOUR POLYMER OR SOLVENT: Carcinogen? Flammable? Toxic? ]

After you submit the ESAF, be sure to get in touch with Ray Ziegler. He will request that your badges be added to the access list so that your badge will work with the proxcard reader to open the door for you.
